The Registrar maintains and provides copies of vital records for the Village of Roslyn.
DEATH RECORDS
Records for persons who died in the Village of Roslyn may be requested by the immediate family (spouse, children, parents) or by other persons who can document a legal or medical need. Applicants applying in person should bring their photo identification, proof of relationship to the decedent, proof of need, and the appropriate fee of $10.00 per copy to the Village Clerk’s Office, Village Hall , Roslyn New York 11576. Applicants will be asked to complete the "Application to Local Registrar for copy of Death Record" form. (see link below)
A request for Death Records may be accepted in writing with the appropriate fee under the following conditions:
1. The applicant is qualified as stated above, and provides the same information requested as when applying in person.
2. The applicant must also provide the name of the deceased and the date and location of death.
