General Information About Online Forms

Forms may take one of three formats:

  • A regular form you fill in and submit online.

  • A PDF form which you print out, complete, and mail.

  • A Microsoft Word document.


Please note, a browser plug-in is required to read a PDF document. You can download the Adobe Acrobat Reader for free if you don't already have it.

Typically, a document requiring an exact format would be furnished as a PDF document. An example might be an Employment Application form. Print a copy of the document once it is displayed by your Web browser. The form will contain specific instructions for completion and submission.

*Please note that most permit and license applications are multi-part color coded NCR forms. As such, they are not suitable for on-line use.

Many Village forms are multi-part, NCR (non-carbon reproduction) and are not suitable for online or download use. Those that are suitable will be listed below. Additional documents, maps and other links are available in our "publications" section at the link below.

Historic District Board

Application must be made for any exterior work performed within the Historic District. The Village has developed a set of guidelines for various types of work which must be reviewed by the Historic District Board (HDB). The Guidelines as well as the application for the HDB may be found through the link below. If you are not sure if the work you are planning requires an HDB application, contact the Building Department at during business hours.
